HIPAA - "Hidden to protect your privacy"

    I have a form located here: https://form.jotform.com/92548477778176

    The form is set to autorespond to the client's email with an attached pdf of the forms information so that they can have a copy of what was filled out. I also have a list of email recipients in my office that need to get a copy of the form data. Instead of getting form data, I see a lock icon and a "Hidden to protect your privacy" but no information at all. So the question is, how am I supposed to have the client who is filling out this form receive their own information and our office staff see the information now that I upgraded to the HIPAA servers?
