This message appears when I try to download an image the was uploaded to a form. How can I view the image?

Have you embedded this form somewhere? Can we have the webpage URL to where this form is placed so we can check?
I have just tested your form and the file has been uploaded just fine. The reason might be something related to the user's end.
Let us know if that will happen again so we can investigate this further.

I've only sent the link out to people through emails. I received a second response and the image downloaded fine.
The small icon next to the person's image upload looks broken, does this have something to do with it?

The file is probably not uploaded correctly and that's the reason why it shows a broken icon on your Form Submission page. I advice you to ask the user to re-submit the file and see if you still can't download that.
Again, let us know if this will happen again on new form submissions.
