How can I create a Soccer Banquet SignUp form with payment included

Hi Support Team,

I would like to create a SignUp form for our high school soccer banquet w Player Name, Attending, Not Attending, # of guests.
Depending on the # of guests, $ amount needs to be paid e.g. $30 per guest. 
Player himself is free.
Payment option needs to be included next.

Could you suggest any of your templates for this?
