The web information posted on Facebook is pulling incorrect form link

The web information when posted on facebook or sent via text message is pulling incorrect information on the title. I am attaching a screen shot from facebook. The -1/2 Grade Boys only portion is from an old form that we copied and modied. I cannot find any place this information is so that I can get rid of it. Can you assist?

I can see that you have updated the Page title in the Form settings. However, in some cases, you need to refresh the information in Facebook. This can be done using the next tool:

I've done this for your form and the form link shows the correct title. Please check if you are still viewing the old title.
