How to change PDF document file name to download?

Hi Team,

How can I change the download name of a form so each time I download the file will save as this 2019-2010 NAME Waiver with Medical Information? Right now when I download the form has NAME and Date anyway to change this (?)

To clarify, do you want to download your submissions as PDF?

You may edit your PDF file name in your PDF editor https://www.jotform.com/pdf-editor/51677075317965 then go to layout settings>PDF file name

Then you can download the PDF document.
