Extra Characters in Jotform Header

I believe there is a problem with the headers in my jotform account.  For some reason many special characters have been added to the header and even if I remove the header and create a new one they are continually added.  

I was able to replicate this in your form. However, when I cloned the form, I was able to see that error again. 

But, when I created a demo form in my account, I did not see the error.

I think the issue is related to the theme used in the form. When I changed the theme, the issue was resolved.

Hence I would request you to try using a different theme in the form.
