Configurable List Year is not working

Having an issue where my 'configurable list' widgert refuses to let me select the year. In fact upon submitting the form does not input the date in the PDF.

I checked your form but I could not check/test the configurable list widget on the form because the section was hidden.

You have hidden the fields below the collapse section. But the collapse section is close and hidden.

You may need to correct this also so that the fields/widgets can be seen.

I was able to check further on your form and I see that the Configurable list widget Date field is working properly at this time.

Thanks, also working for me today. It wasn't working on Friday but seems to have resolved itself. I saw a bunch of errors on Friday with PDF forms so maybe it was all related at the time. Whatever happened over the weekend has resolved itself?
