Form Layout Issue in different browser

Hi,

I have created a form please check http://intomesee.com.nmsrv.com/join-us/ found layout issue if i browsed that form on different browser like chrome, Firfox. can you please check and let me know how i can fix that issue

I have checked your web page and it seems you have embedded two forms in your web page.

I checked your web page in FireFox and Chrome but I can't seem to find any difference in the layout.

Can you please share a screenshot on the layout issue?
