Suggestion for template

Hello,

I own a business that sells a specialty type of antique for people. I have questions about their name, if they are the legal owner, the size, how long they have owned it, the size, if they have tried to sell it to others and how, etc. These are all for the most part in drop down menus. Some are yes or now, some give a little space for them to type in specifics, and the rest are drop down menus with 2-5 choices.

You know your forms well I am assuming. Can you suggest 2-3 that might be appropriate for my web site and customise the questions and drop down answers, or do I have to create my own form?

Because the form you are looking for is very specific (antique), there is not much similar users contributed form template in the form template gallery.

But there are Evaluation forms you may want to check also.
