I am able to view the spreadsheet you have shared.
It is possible to use math functions with the use of the Form calculation widget.

However, can you please share more information on how you would like your form to work? Do you want your form to be like the spreadsheet you have shown?

Yes, I wish I could just embed this spreadsheet into a form. Is that possible?
