When I copy the source code over into my web page, some of the fields are not aligning the same way as they are while I am creating the form. For example the Birthdate, gender and my terms box at the bottom of the page. Any suggestions.

There seems to be a script conflict with the Source Code and the other codes that you have in your blog. I would suggest you to switch to embedding your form to iFrame embed code instead to prevent script conflict. Thanks a lot; that worked and my form also looks a lot cleaner as well as the form is centered on the page!
