I checked this link via Chrome on web browser and was able to see the image added in the Header field.
I checked the same from my mobile device and again I was able to view the image.
Are you still facing an issue? Have you checked this via a web browser or on your mobile device?

Yes, I have and its still the same, without the header image.
I've also cleared cache both on Jotform and on my laptop as recommended on other support tickets.
All yielded no positives. What else can I try?

I am also able to see your image on the form.
Can you please try to use different browsers and a different laptop, if still not working? Did you try to open it on mobile devices?
It looks like that the issue is related with your devices.
Please test again and let us know, if it helps.
