Why aren't my questions being extracted into my PDF's

    Hi team,

    Whenever someone completes a form we download a PDF of the answers and the questions on the form are not coming out correctly on the PDF. For example, I have downloaded our Investment Management form which a client has completed and it has the clients answers but they are under completely different questions. However, when I extract it in Excel or CSV they are correct. I have taken a screen shot of part of the document so you can see what I mean. 


    Do we have to set up the PDF so the questions come out correctly or is this a coding error from your end?

    Could you please delete the current PDF document and create a new one? You can click on the Customize PDF document and create a new PDF.
