My account says it is suspended when I try and log in

So I posted this yesterday, but I can't get back to the page, tells me to log in, but when I try and log in with the email I used in this form it says suspended, which is the problem I was trying to fix

When I posted yesterday I was told the email was a temporary account or something like that which it isnt.  I've used this for a couple years.  My work's IP was banned for potential spam issues (not sure why), I filled out the form and you all stopped blocking it after seeing it's a high school

Can you please share your account username or the email address used in your account so that we can check it out?

I don't think I have a user name or I dont know it.  I'm not sure what a guest account means.  I have an account created with this email

I was able to locate your account jtrand02 and it seems to be suspended as suspicious of phishing. It seems your form is collecting ID number which is a sensitive information.

If you want to collect ID number, you need to add the Terms & Conditions and Signature widgets to your form, for users to confirm/allow/agree that you are collecting the information for a purpose.

The ID number is a school ID number if that makes any difference, every student has one and I have access to them, just easier if they tell me what it is

I have removed the Id number field and lifted the suspension in your account.

Kindly add the two widgets if you wish to add the ID field again to avoid your account getting suspended.
