I am trying to test my form and every time it says the card is declined

However the form is set to Stripe in test mode.

While trying to test the form with the Stripe payment in test mode, try using the test cards that Stripe has provided: https://stripe.com/docs/payments/cards/testing#cards 

I can see that you have now changed the mode to Live. Alternatively, you could test the form using a sample amount as 1 USD and test using your actual card. If payment goes through successfully, you can also refund that payment back into your card via your Stripe account.
