Why is my account suspended?

I have tried to log in and it is telling me that my account is suspended? The username is urbanmutt. Can you please let me know what the problem is? We have not received any emails about this.

I have checked your account and it was suspended due to the credit card fields on the form https://www.jotform.us/form/92813785855169.
