This form disapeared between december and now. I put a lot of time into the form and would like it recovered.

I never embedded it, that's the problem.

on this page, you can see the first item, the blue banner, order item was tied to the form, it was going to pre-populate some fields.

Regarding the comment below, I am a Premium user of 1 account gdavis (at) eqrworld.com, and am turning several accounts to premium. The creation of the form I'm looking for and coding of my site to prepopulate the jotform took about 2 hours, I'm not sure how to take the comment below. This is preventing me from becoming a Premium user for this acccount and others. I've recommended JotForm to about 5 people. I realize it's not fun to dig through backups. It's not fun to have your work just vanish either. I can tell you if this form gets restored, this client has no option but to become a premium user.
Since recovering a form from backups is 20-minute manual work, we can only do it for Premium users.
