Unable to submit form since only one entry is allowed.

    Hi, 

    I have created two forms for my clinic Website so New patients can fill it out and submit it from the website. Both are HIPAA Compliant. Both forms are located on website with very clear instructions to fill both forms. When someone fills out both forms only one form is being submitted depending on which forms if filled out and submitted first and after submission of second form instead of showing Thank You you form has submitted successfully it says Only One entry is allowed.

    Please let me know where is the problem and how we rectify it.

    That error is shown, because the form only allows one submission per persons IP address: https://www.jotform.com/help/64-How-to-Set-Unique-Submissions-on-a-Form

    Please select No Check to allow multiple submissions. Let us know if you have more questions, we will be glad to assist you.
