Form not found | Account is suspended

    Hi! We have a form ongoing about a week now but just now we encountered two errors.

    Firstly, It says that "the form is not found. The form is disabled." This can be shown attached.May I know what does this means?

    Also, we couldn't log in to our account. It says that the account is suspended. This can be shown as below.

    Online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting ID numbers is considered as phishing activity, as stated in our Terms of Use. Our phishing detector will automatically flag the forms found and suspend the account or this can be manually performed by our Staff.

    If your purpose is not to collect these data from the general public, but from your own customers, then you need to comply with two things.

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    However, we do not allow uploads of any sensitive personal information like passports, social security cards, driver's licenses, credit/debit cards and statements, bank statements, voided cheques, birth/marriage certificates, visas and photo IDs.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    In order to activate your account, it is required for you to agree with our terms of usage and agree to delete the guilty fields on the form. Please confirm so that we can delete the guilty fields on the form and activate your account.
