My account was autosuspended

    Hello, my account was auto suspended. I need access to my account, could you please help?

    Hi Kierdre,

    Upon review of your account, it was found that form id: https://www.jotform.co/form/92887000358865 has fields asking for Bank information, the reason why the account has been auto-suspended.

    Collection of sensitive banking information are allowed provided that you will add the following mandatory widgets

    1. Add the Terms & Conditions Widget(or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information. (already present in your form)

    The widgets will apply for bank Information collection. This will make you the sole responsible for the data collection and handling.

    I have reactivated the account for you to be able to add the said widgets. It will be auto-suspended again in case you didn't comply with the said requirement.

    Hi there, 

    I added the terms and conditions widget, exactly as asked above, and was autosuspended again. 

    What am I missing?

    I have reactivated the account so you must be able to login now. 

    Also, please add a valid link to a webpage containing your Terms statement. If you have none, you can use the Scrollable Terms widget instead and add your Terms statement there:
