My site is a pro Weeby, so I'll need to paste your HTML code in. Any probs??
I run a club, with an increasing membership and there is a move to make things like the newsletter (read on line option) and now membership renewals, Electronic. This saves postage, effort, resources.
Not happy with Weebly form set up, too restricted eg, "zip code" is "post code" in Australia, but I can't edit the Weebly address form to reflect this.
I need to ask members questions to update my data base (records), but again it's difficult in Weebly.
At present, there are a number (10's ) of members that will use the system, but what's the limit?
It will be used for 3 months, once a year, but left dormant on site till next subs season. Is this software "mine" once it's on my site?

There's no problem for as long as your settings are correct. Check this guide on how to add Form to Weebly: http://www.jotform.com/help/215-Adding-a-Form-to-Weebly.
Newsletter: You can integrate your form Mailchimp, Aweber, Constant Contact and many others.
Membership (Recurring Charges): You can add our Payment Tools like Paypal.

Limit: You can receive up to 100 submissions per month, total on all forms within your account. If you want more than this per month, the best option is to upgrade your account to our Paid Plans which will give you up 1000 submissions per month.
Yes, the form is yours for as long as you won't delete them from our website.
