Why my account is suspended?

    My account has been suspended and I am no longer able to access the forms, thank you to unlock my account

    I did check your account and found that it is currently suspended because of the ID or Passport upload field you have added in forms.

    Phishing. You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    However, you can still collect ID and Passport Numbers using the plain text fields instead of file upload and only allowed when you added mandatory Terms and Conditions and Signature widgets.

    I would also suggest you change your account email address to a corporate one.

    For now, we can delete the file upload field and reactivate your account. Once we reactivate, you can use a text field to collect ID Numbers and add the required widgets. Also, you will need to use a corporate email.

    I have removed the field my colleague is referring to on both of the active forms in your account and reactivated your account.

    You may refer to the following guides when adding the Terms&Condition widget and Signature widget in your form.
