Collecting Supporting Documents or Files

i tried reaching out a few days ago with no answer.

is there a way to collect documents or upload documents very similar to below where we can get the client to choose a file to upload?

how do we do it and what integrations do we need?

In answer to your question, yes you can use the form to let your form users/respondents to upload documents. You can use the Upload field for this.
