How to submit filled-up form?

My client already filled-up the form up to the end anf clicked submit, but it only redirected to the 1st page again. Why is it not submitting? and did not notify or sent to my email?

Is this form the one you are referring to Pet Boarding Form ? As I check, it seems to be correctly set up. Would it be possible for you to ask the user try again? I assume there could be a connection interruption or something similar.
