    2.  We use GSuite.  I got all of the information  (SMTP, port number) from them but when I sent a test email, but it still came from JotForm.  Please help.

I have checked your account, but I cannot find an SMTP sender, please kindly set it up following this guide: https://www.jotform.com/help/244-How-to-Setup-SMTP-for-a-Form

If you have questions, let us know.

I think I've created it on both forms now (thank you), but it won't let me edit the sender's name or the reply to email.  When I click on the drop down box, it gives me parts of my form to select, no the company name.

I want the emails coming from Work Well, PLLC.  Please help me do that.

While using SMTP, the sender name will change to the one configured on your email server, but the reply-to email can be changed from the email settings:

The drop down will display fields from the form since the purpose is to reply to the submitter; however, in case you would like a fixed email address as reply-to, you may add a hidden email field and configure the desired address as default value, then select this hidden field on the "Reply-to Email" section on the email wizard.
