Why is my account suspended?

I am working with Turhan Berne. We just signed up an account for gptapros2019 and made our first form.

The form was published and we were able to fill out three submissions. Then, the form was disabled and our account is suspended.

What can we do to enable the form and make the account active again.

Any help would be appreciated ... especially today.
