Can HIPAA complaint Prescription forms be made in JotForm?

I am wondering if you guys have a form or 2 for prescription model for both human and pet HIPPA compliant

1) Upgrade your account to Silver or Gold: https://www.jotform.com/help/331-How-to-Upgrade-My-Account

2) Then upgrade to HIPAA: https://www.jotform.com/help/500-How-to-upgrade-to-HIPAA-Compliance

In regards to HIPAA templates, you can search in our templates gallery https://www.jotform.com/hipaa/templates/

If you don't find one that would meet your requirements you can build the form from the scratch.
