I am not sure I understood your question correctly. Could you please explain a bit more so we can assist you better?

You can set up the email notification to get the email when someone submits the form.

Or do you wish to show some fields only if the user passed? If yes, you can use conditions for that.

    1. i pulled in a text box to type the question ; if you know someone that has passed..... but it doesn't show the question (on the left) i typed (only their answer to my question ) when i get the copy of the form in my email once someone submits it..

    2. My other question is..  where ive typed - submit a business or service you want to share/ promote...

how can i get that text to go all the way from left to write across vs. how it is only on the left side like a paragraph, it wont go all the way across it returns to the next line right away .. i want it to go across like this (i just put periods to show i want to stretch  it across) for example:

submit a business or service you want to share/ promote..........................................,.....

    Not like this:

     submit a business

    or service you want to

    share/ promote

    1) Could you please confirm you are referring to the text field below?

Do note the text will not be displayed in the Notification email automatically. You will need to manually add all the text in the email template.

    2) You can always edit the email template and customize it as you wish.

Yes, you need to add the texts, images, and headers in the form emails manually. There is no option you can tick off to automatically add these elements in the template. Kindly check the guide shared by my colleague to do that.

You can use the table tools to add rows in between fields:

You may also consider creating a PDF version of the submission and attaching that in the form email. That way, you'll get it as PDF instead of modifying the email template.
