I reviewed your account/username simplyfrosted and I noted that is in Overlimit Status due to exceeded Upload space limit.
You can also review the details in your my account usage page https://www.jotform.com/myaccount/usage
To restore the forms and account, you need to :
1. Upgrade to the next higher plan to have expanded Upload space.
OR
2. You can delete submissions that have submission uploads to reduce the Upload space usage.
