Some of my widgets stop working when switching Form Layout

Hi There, 

I want to convert my form layout for the "delivery request form" onto a one page layout, without loosing all the functionality. When I try this, some of my widgets ect, stop working.

Thank you for contacting us. Please note that some widgets are only supported in a specific layout.

So, when the layout is switched, you will need to adjust the form, and replace those fields that are not supported, example:
