Custom Form Creation Cost

I noticed there was an option to "Request a Form".  I'm curious what the cost is for this service and what if any constraints exist.

It would not cost you anything to request a form. If you made a valid request, your request form will be most likely created. Once it is done, you will receive an email about it.
