I am getting message saying that the form is disabled... how do I enable it?

Your form seems to be active now. One thing I found is that you have enabled Unique Submission on your form. I suggest you to disable that or please kindly check this guide for more information: http://www.jotform.com/help/64-How-to-Set-Unique-Submissions-on-a-Form.
