I am wanting to build a Order form wherein the customer could choose first their Carrier (i.e Verizon, Sprint, AT&T) then it give them options, based upon the carrier they selected, to order prepaid Refill Cards (i.e. $25, $30, $40...etc) then finally to select the quanitity that they need. How can I do this? I want it so that if they select Verizon as the carrier, then only Verizon Plans are displayed.
I think that's possible with JotForm with Conditional Logics. You can hide fields depending on what the user selected on the first part. But, since you can only use 1 payment field type on every form, you have to create all the products/plans there. After that, get your form source code and use custom scripts to hide/show the plans depending on user's selection.
