How can I create a form with recurring subscription?

Provides subscription products that can be purchased monthly for $X or yearly for $Y without having to create two separate subscriptions?

Using this option you will have to create one subscription as yearly and a second subscription as monthly in the same payment field. 

 

You cannot create the monthly and the yearly payment option inside one subscription.
