How do I activate non working check box questions?

    I have a form named 'Request for Information.' It was recently updated. Questions termed "Student Sports Interest" and "Student Activities Interest" are now check boxes. They were radio buttons.

    Unfortunately, my new questions aren't providing information. I performed a test. It follows. Both sections should have some test info, but they are blank.

    Comments/Request: This is a test of the new features. My modifications don't seem to be working.

    I noticed that these fields are not marked as required. If the user did not fill
    out these questions it will not be shown in the submission altogether with the
    labels. It will just skip to the next field that was filled out.

    I also checked the notification emails for the form the Notification2 alert has the
    following variable in the body text:

    Student Sports Interest: {studentSports}
    Student Activities Interest: {studentActivities}

    Upon changing the fields from radio button to check boxes the variables were
    also updated. The new variables for these questions should be:

    Student Sports Interest:  {studentSports21}
    Student Activities Interest: {studentActivities22}

    To amend this on the form email please do the following:

    1. Open your form in JotForm
    2. Click on "Setup and Embed"
    3. Choose "Email Alerts" then "Notification 2"
    4. In the Compose Email window the right section shows a list of the form fields.
