Forms to ask for Employee Email Credentials and login passwords

    Good Day!

    We would like to ask if its allowed for us to make a form and ask our employees log in credentials.

    Like this form below:

    And we are going to instruct them what should they input per number.

    Example:
    1. Email User Name

    2. Email Password.

    Hi @hr.linkservesolutions,

    Unfortunately, we don't allow users to create forms that ask for usernames, passwords, login information, and other sensitive information. Please check our Terms of Use and our Privacy Policy.

    Here's a snippet of the Terms where this is being discussed:

    "Phishing. You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example, sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."

    This means, no matter what the purpose is if our system or we detect that someone is breaking our Terms of Use which if you ask for login details, we will suspend your account.
