adding a link to a document

Is there a way to add a link to a document on the form and give it a title. 

I want to add this to the bottom of a form and I am not sure how to do it

Yes, you are able to add a link to your form by using a "Text Field" on your form.

From the Form Builder add the "Text Field" to your form and then click on the pencil icon to edit.

Next click on the icon to insert/edit a link.

You will get a popup box to paste or enter the link you would like to add to your form and then click on OK.

Now you will see your link that you added on your form.
