How can i create a checklist form?

We want to create a checklist form as per following createria.

- There are two person will work as maker and checker

- Maker will work on the checklist as per his requirement and forwarded to their checker

CHecker will received the notification on his predefined email id and then revalidate the maker task adn then submit

checker can do the comment or provide his remark but can not change the maker remark/commect.
