Request for completion of Vendor Risk Assessment

I'm illias from Certis Cisco. 

I have been using your service for quite some time now under my account cpetpehdb.

My company is considering adopting the usage of Jotform for form development too.

However, in doing so, we would require Jotform to answer a list of questions as part of our Vendor Risk Assessment.

Would you be able to provide a contact person for me to link up with, so that I can direct the list of questions to him/her.

We request you to forward the assessment to support@jotform.com along with the plan that you may want to upgrade. Generally, the assessments shall be addressed for the users upgrading to Gold plan. If you are looking for enterprise plan, we request you to contact our Enterprise support by submitting the form in our enterprise web page.

We have yet to decide between either Gold plan or Enterprise plan.
For the sake of the assessment, can we regard it to be for Gold plan first.
