Hi All!
I have an existing HTML form that I would to convert to a Jotform.
Cam I possibly just add additional code to the HTML form to make it submit
the form data to my Jotform account?
The form is in the footer area of the page linked below.

I'm sorry, but you cannot do that unless you know how to hard code the form and POST URL. You may want to create the form through Jotform and embed it into your website.
The form is simple and can be mimic with custom CSS codes. We can help you get the custom CSS codes to inject in the form. Just create the form and let us know the URL.

I am OK with adding the form and POST URL code...can you provide some sample code?

I'm sorry, but I realized you could not just get the POST URL and put it in on your existing form. It is just too technical to say, and I'm afraid it is out of our scope of support. I would suggest replacing the form on the website with Jotform.
Here's a starting form you can check: https://form.jotform.com/93294383799982. I'll work on the design and let you know.
