Yes/No Question value is not changing after I updated it 3 times

Question 5 of my Post An Item survey is not changing. I keep on changing No to Email and it is not changing. For question 5 the answers should be text or email. Please look into this for me.

The form is embedded on my website, post an item page www.myflipitassistant.com . Ever since the edit, people cannot hit the submit button at the end of my survey that is embedded into my website. It works on the jotform website but not my website embedding.

We apologize for the inconvenience caused. I also could not reproduce the problem when check/test the form embedded on your website.

But it could be possible some users were getting script conflict related issue on the browser when using the form on your website due to the wp oembed code of the form.

What I suggest you try also is to re-embed the form on your website using instead its iframe embed code.

Make sure to remove/clean first all the form's previous script codes used on the website before you re-embed using the iframe code.

Do let us know if using the iframe embed code still did not resolve the issue.
