Why my account is suspended?

    When I try to login jotform,  error is occurred with the message "Login failed - SUSPENDED". 

    Please help to check the problem. Thanks.

    As checked, your account has been suspended by our system as these forms:

    contains plain text fields that collect ID/Passport numbers. It is considered as phishing and violates our Terms of Use

    You may collect passport information if your purpose is not to collect these data from the general public, but from your own customers, you just need to comply with two things:

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    We need to delete the infringing fields to lift the suspension. Please advise if you agree so we could proceed.

    Once reactivated, please add the above widgets to prevent account suspension in the future.

    I have removed the infringing fields from the said forms and reactivated the account.

    You may now login to the account again.

    ok, thanks.

    I still have one problem. When I load jotform page using my PC, it show “Looks like your IP address has been banned”. It’s ok for me using mobile to load jotform page.
