How can I create a form for candidates to select a date for telephone interview with options disappearing once selected?

I want to create a form that has pre selected dates and times for candidates to choose from, once a candidate has selected a date and time slot it should then not be available by another candidate to select.

You can use the Quantity Gift Registry widget. You can list all available dates and times in the widget. When the items on your list have run out, the widget will cross out the item as unavailable.

The Quantity Gift Registry Widget is good for forms that have a limited list of items that need to be picked or selected. You can find the widget here:

I have selected the widget and when inputting the date and time the format changes and is unreadable. 

I also get the following error message when trying to preview the form.

The server encountered a temporary error and could not complete your request.

Please try again in 30 seconds.

I checked your form and was able to add more option and preview. You can see the options, which I had added on the form.

In order to fix it, can you please try to clear cache, cookies and recent history in your browser and use different browser, if still not working?

Fantastic, I've added in the dates and times I needed. The only issue now is the green text that say "1 left" is, in some lines only, sitting behind the black text and hidden from view?

You can edit the "left" label from the settings of the widget. You should click on "Widget Settings" and then edit the "Counter Label" field. Please refer to the attachment.

You can also adjust the available quantity of the items. You should just enter different value in the brackets.
