Multi-stage Approvals and tracking?

Hi, is there a template for a form that requires multi-stage approvals and tracking?

We have an Original Resource Request Form.  The teacher submits a request to the VP for approval.  If approved, it comes to my desk.  Upon completion, I detail what was completed, when it was completed, who completed it, and how much it cost if anything.  it then goes back to the VP for approval.

These forms are to help with data collection.  At the end of the year, we can tell funders what, where, who, why, when, and how much.  Is there a way to track certain fields?  For example, how many requests came in from the Grade 3/4 room.  Or, how much money was spent on deliverables.

Any suggestions would be greatly appreciated.
