Hello,
When users complete and submit one of our forms, specifically the Participation Agreement for Youth, a PDF copy of the completed form does not route to the designated email recipient. I have tried to adjust the settings, but the PDF of the completed form does not appear to be attaching to the email alert. Can you review the settings for this form and let me know what I need to change? As far as I can tell, when users complete and submit other forms, their alerts route to a designated email recipient with a PDF version attached.

As of checking, none of your 3 email alerts have PDF Attachment enabled.
You have to enable the PDF Attachment option in the email notification first in order for the PDF to be attached to the email. Here's a guide: https://www.jotform.com/help/503-How-To-Include-Submissions-As-PDF-Attachment-In-The-Email-Notifications
