Hi there,
I would like to ask, I have a problem reverting to my previous edited version. I have tried numerous times but still failed. It is quite urgent can you assist?

I have tried this and it is not working. I have tried clearing caches and cookies and tried on both Microsoft and Google Chrome browsers, and it will not let me revert back to earlier form history. it says it has reverted but keeps using current form.

Which revision you want to revert to?
I see that on Dec 2, 2019 there are two revisions where one is the current version and the other one has 6 updates. I have reverted your form to the Dec 02, 2019 with 6 updates.
Please check if the form is reverted correctly.

I would like to revert to the 27 Nov, 4.14 PM version. Please find attached a screenshot of how the version should look like.
