Why when I log into the computer jotform there’s nothing there?

    I only can see my forms and submissions on my iPhone

    I can see that there is only one form in your account titled "Client Medical History, Policies, Waiver/Consent Form".

    It seems that you may have logged off from the JotForm account on your computer. Please log back in and you will be able to view your form.
