Hi, I uploaded a pdf to convert to an embeddable form and something is worng?
Why is it making my pdf look all messed up? Can someone please see the form in my account?

This is how I see that form in your account.
Could you please explain with a screenshot what is not looking fine? You may try re-uploading the PDF file and see if that fixes the issue. If not, please share the PDF file with us so we can check.

Please share a screenshot of how the form is looking at your end. Also, could you please upload the PDF document to google drive and share the link with us, so we can try at our end?

I can see that there is an issue while uploading the PDF. Please confirm if you followed this guide to import the PDF form: https://www.jotform.com/help/548-How-Can-I-Import-My-Own-PDF-to-JotForm
If yes, could you share your original PDF so that we could try to replicate the issue from our end too.
