How can I require a password/login to access my form?

    I would like my form to create a profile for a user and require a login to access and update the information.  Is this possible?

    Hi Gordon,

    I'd like to inform ou that you it is not advisable to create a form that will require providing info for username, password, and combine with email.

    You cannot do combinations like this:

    username/password

    email/password

    username/email/password

    If you use this on your forms, there is a 100% chance the phishing system will suspend your form.

    ---

    You can create a user profile providing only username/Name and email.

    If you want the form to be edited by user later, then I suggest using Condition logic, and provide the user with the info on how they can open the form. You can add a hidden field or unique ID field that users will be provided a value where they can use it to unlock the form(show fields)

    Something like this http://form.jotformpro.com/form/30925350328956?password=testcode  instead of password use hidden box with value.
