    I want to assign a certain numerical value to each of the selections: no problem = 0, Very mild problem = 1, Mild or Slight Problem = 2, Moderate problem = 3, severe problem = 4, problem as bad as it gets = 5.

    Is your question related to the Tell Your Story form? I inspected the form and the calculation values are assigned properly.

    However, your calculation is not set properly. You need to add + sign after each field to perform a calculation. You need to set it like this.
